Application for Examination
This is the form you will need to fill out to apply to take the Oklahoma Optometric State Board Exam.
The examination fee is $200.
Please note: Your ORIGINAL transcripts for your Pre-optometric and Optometric Education are needed to complete your application as well as a copy of your current CPR card.
