District Attorneys

1.	Complete an approved firearms training course conducted by a certified firearms instructor
2.	Send the District Attorney Firearms Qualification Report. This form has 2 parts
	a. Part 1 – to be completed by District Attorney
	b. Part 2 – to be completed by a certified firearms instructor
3.	Send proof of your employment as a District Attorney.

DA Firearms Qualification Report

Assistant District Attorneys

1.	Complete an approved firearms training course conducted by a certified firearms instructor
2.	Send the Asst. DA Firearms Qualification Report. This form has 3 parts
	a. Part 1 – to be completed by the Asst. District Attorney
	b. Part 2 – to be completed by a certified firearms instructor
	c. Part 3 – to be completed by District Attorney
3.	Send proof of your employment as an Assistant District Attorney.

Asst. DA Firearms Qualification Report

Retired District Attorneys

1.	Complete an approved firearms training course conducted by a certified firearms instructor
2.	Send the Retired District Attorney Firearms Qualification Report. This form has 2 parts
	a. Part 1 – to be completed by District Attorney
	b. Part 2 – to be completed by a certified firearms instructor
3.	Send proof from the appropriate retirement system verifying your status as a Retired District Attorney.

Retired DA Firearms Qualification Report

U.S. Attorneys

1.	Complete an approved firearms training course conducted by a certified firearms instructor
2.	Send the U.S. Attorney Firearms Qualification Report. This form has 2 parts
	a. Part 1 – to be completed by U.S. Attorney
	b. Part 2 – to be completed by a certified firearms instructor
3.	Send proof from the appropriate retirement system verifying your status as a U.S. Attorney.
